Atualmente, novos medicamentos ainda est\u00e3o sendo desenvolvidos.<\/p>\n\n\n\n<p><strong>Como esses anticorpos funcionam?<\/strong><br>Basicamente, quando o anticorpo se liga a uma c\u00e9lula cancer\u00edgena, ele pode:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Tornar a c\u00e9lula maligna mais vis\u00edvel para o sistema imunol\u00f3gico;<\/li><li>Bloquear sinais de crescimento;<\/li><li>Impedir a forma\u00e7\u00e3o de novos vasos sangu\u00edneos que fornecem oxig\u00eanio e fatores de crescimento;<\/li><li>Enviar radia\u00e7\u00e3o \u00e0s c\u00e9lulas cancer\u00edgenas: neste caso, as part\u00edculas radioativas est\u00e3o ligadas ao monoclonal, permitindo baixos n\u00edveis de radia\u00e7\u00e3o por um longo per\u00edodo de tempo;<\/li><li>Enviar quimioter\u00e1picos \u00e0s c\u00e9lulas cancer\u00edgenas: como acima, ligando as drogas quimioter\u00e1picas.<\/li><\/ul>\n\n\n\n<p><\/p>\n\n\n\n<p><strong><em>O Daratumumab (DARA)<\/em><\/strong><br>O medicamento Daratumumab (DARA) \u00e9 um anticorpo monoclonal (anti-CD38) utilizado no<br>tratamento de pacientes com Mieloma M\u00faltiplo. O DARA tem como alvo o ant\u00edgeno CD38 que \u00e9<br>altamente expresso nas c\u00e9lulas com mieloma, tornando-as mais vis\u00edveis ao sistema imune.<\/p>\n\n\n\n<p>O CD38 \u00e9 fracamente expresso nas hem\u00e1cias normais. Assim, o anti-CD38 liga-se ao CD38 nas<br>hem\u00e1cias-teste, causando panreatividade in vitro. As amostras dos pacientes em tratamento<br>com o DARA apresentam rea\u00e7\u00f5es fracas nos Testes de Antiglobulina Indireto (TAI): Pesquisa e<br>Identifica\u00e7\u00e3o de Anticorpos Irregulares e Prova Cruzada.<\/p>\n\n\n\n<p>A aglutina\u00e7\u00e3o devido ao anti-CD38 pode ocorrer em todos os meios (como salina, LISS,<br>polietilenoglicol), e com todos os m\u00e9todos em AGH (gel, tubo, fase s\u00f3lida). Rea\u00e7\u00f5es de<br>aglutina\u00e7\u00e3o causadas por anti-CD38 s\u00e3o geralmente fracas (1+), mas rea\u00e7\u00f5es mais fortes (at\u00e9<br>4+) podem ser observadas em algumas metodologias.<\/p>\n\n\n\n<p><strong>Como o DARA interfere nos testes de AGH:<\/strong><\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-full\"><img fetchpriority=\"high\" decoding=\"async\" width=\"526\" height=\"460\" src=\"https:\/\/bio-rad.com.br\/imunohematologia\/wp-content\/uploads\/2022\/05\/Sem-titulo.png\" alt=\"\" class=\"wp-image-2261\" srcset=\"https:\/\/bio-rad.com.br\/imunohematologia\/wp-content\/uploads\/2022\/05\/Sem-titulo.png 526w, https:\/\/bio-rad.com.br\/imunohematologia\/wp-content\/uploads\/2022\/05\/Sem-titulo-300x262.png 300w\" sizes=\"(max-width: 526px) 100vw, 526px\" \/><figcaption>Fonte: Janssen Biotech, Inc. 2015. has-black-color\">Descri\u00e7\u00e3o de caso:<\/mark><\/strong><\/p>\n\n\n\n<p>O LAC recebeu 7 amostras de pacientes em tratamento com o Daratumumab entre 06\/2016 e<br>12\/2017.<br>Os pacientes interfer\u00eancia do anti-CD38 pode mascarar a presen\u00e7a de aloanticorpos clinicamente significantes.<\/p>\n\n\n\n<p>Para eliminar a interfer\u00eancia do DARA no painel de identifica\u00e7\u00e3o de anticorpos irregulares, uma solu\u00e7\u00e3o simples \u00e9 o tratamento das hem\u00e1cias-teste com DTT, que destr\u00f3i o ant\u00edgeno CD38 evitando a rea\u00e7\u00e3o com o anti-CD38 para ent\u00e3o, realizar os TAIs.<\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large is-resized\"><img loading=\"lazy\" decoding=\"async\" previamente \u00e0s transfus\u00f5es, as pesquisas imuno-hematol\u00f3gicas, como tipagem ABO, RhD, pesquisa de anticorpos irregulares, fenotipagem eritrocit\u00e1ria, e, eventualmente a genotipagem, para que posteriormente n\u00e3o ocorram atrasos no atendimento hemoter\u00e1pico, al\u00e9m de evitar erros na identifica\u00e7\u00e3o dos anticorpos irregulares por interfer\u00eancia do DTT destr\u00f3i o ant\u00edgeno Kell, unidades K-negativas devem ser fornecidas, a menos que o fen\u00f3tipo do paciente seja conhecido previamente como K-positivo.<\/em><em><\/em><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li><em>N\u00e3o pode ser afastada a presen\u00e7a dos anticorpos contra ant\u00edgenos dos grupos sangu\u00edneos que tamb\u00e9m s\u00e3o desnaturados ou enfraquecidos pelo DTT 0,2M, como o <strong>KELL, GE, LW, YT, KN<\/strong>, <strong>DO<\/strong>, <strong>LU<\/strong>, <strong>JMH, IN<\/strong>, <strong>CROM.<\/strong><\/em><strong><em><\/em><\/strong><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li><em>Os estudos de adsor\u00e7\u00e3o utilizando tanto hem\u00e1cias tratadas por ZZAP quanto n\u00e3o<\/em> <em>tratada , falham na remo\u00e7\u00e3o do anticorpo.<\/em><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li><em>Algumas c\u00e9lulas raras com fen\u00f3tipo Lu(a-b-) n\u00e3o s\u00e3o reativas na presen\u00e7a do anti-CD38, o que potencialmente pode fornecer a falsa impress\u00e3o de que o paciente tem um anticorpo relacionado ao sistema Lutheran.<\/em><em><\/em><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li><em>TAIs positivos podem ser observados por at\u00e9 seis meses ap\u00f3s o descontinuidade do anti-CD38.<\/em><em><\/em><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li><em>O anti- D38 pode causar uma pequena diminui\u00e7\u00e3o na hemoglobina in vivo (~ 1 g\/ dL), mas hem\u00f3lise severa n\u00e3o foi observada entre os pacientes em tratamento.<\/em><\/li><\/ul>\n\n\n\n<ul class=\"wp-block-list\"><li><em>Como alternativa, a prova cruzada em AGH pode ser realizada usando hem\u00e1cias
